Medical Justification Template

A structured letter template your physician can adapt to document medical necessity — built around the language coverage reviewers are actually trained to look for.

How to use this template

Step 1

Share this template with your treating physician — this letter needs to come from them, on their letterhead, not from you directly.

Step 2

Fill in the highlighted fields together with clinical specifics — vague language is the most common reason justification letters get rejected.

Step 3

Attach it to your internal appeal submission, alongside any supporting clinical guidelines or prior treatment records.

The template

Copy the text below and share it with your physician as a starting draft. Text in highlighted brackets needs to be replaced with your specific details.

[Physician Letterhead] Date: [DATE] RE: Medical Necessity — [PATIENT FULL NAME], DOB [DATE OF BIRTH] Provincial Health Number: [HEALTH NUMBER] Requested Service/Treatment: [NAME OF SERVICE OR PROCEDURE] To Whom It May Concern, I am writing on behalf of my patient, [PATIENT NAME], to provide clinical justification for the above-noted service, which was denied on [DATE OF DENIAL] on the stated basis of "[EXACT DENIAL REASON AS STATED]." DIAGNOSIS AND CLINICAL HISTORY [Patient's diagnosis, date of diagnosis, and relevant clinical history in 2-4 sentences] WHY THIS SERVICE IS MEDICALLY NECESSARY [Specific clinical reasoning: what symptoms/risks this addresses, why standard alternatives are insufficient, and what clinical guideline or standard of care supports this treatment] TREATMENTS ALREADY ATTEMPTED [List prior treatments tried, dates, and documented outcomes — include why each was insufficient] ANTICIPATED OUTCOME IF DENIED [Clinical consequence of continued denial — symptom progression, risk of complication, or functional decline expected] SUPPORTING EVIDENCE [Reference any relevant clinical practice guidelines, peer-reviewed literature, or specialist consult notes attached to this letter] I am available to provide further clinical documentation if required. Given the clinical urgency described above, I would appreciate an expedited review where possible. Sincerely, [Physician Name, Credentials] [License/Registration Number] [Contact Information]

What makes a justification letter effective

  • Address the exact denial reason stated — a generic letter that doesn't reference the specific grounds for denial reads as templated and carries less weight.
  • Lead with clinical terminology, not personal impact — "activities of daily living are significantly limited by [specific symptom]" outperforms general descriptions of hardship.
  • Document what's already been tried — reviewers weigh a documented history of escalating, unsuccessful alternatives heavily.
  • Keep it to one page where possible — a focused, specific letter is read more carefully than a long one.
  • Attach supporting evidence rather than describing it — a cited guideline or specialist note carries more weight referenced directly than summarized secondhand.

Important: this is a starting template, not medical or legal advice

The Patient Insider is not a healthcare provider, insurer, government agency, lawyer, or patient advocate. This template is a general educational starting point for a physician-authored letter. It is not a guarantee of approval, and it does not replace clinical judgment — your physician should adapt every section to reflect your actual diagnosis, history, and clinical reasoning.

Appeal processes, required documentation, and timelines vary by province and by insurer. Confirm the specific requirements for your case with your provincial health ministry or insurer before submitting.

If this is a medical emergency, call 911 or go to your nearest emergency department now. Do not use this page to delay urgent care.
