Reimbursement Checklist

Everything to track — from your first out-of-province appointment to your final reimbursement — so nothing gets missed and no deadline slips.

Before you receive care

Written pre-approval from your home province health ministrySave the reference/file number, not just a confirmation email
Confirmed coverage rate in writingAsk specifically whether you'll be responsible for any difference between the receiving province's rate and your home province's rate
Referring physician's letter and contact informationKeep a copy for your own records, separate from what was submitted

During and immediately after your appointment

Itemized receipt or invoice from the provider/facilityMust show service codes or descriptions, not just a total amount
Proof of paymentCredit card statement or receipt showing the transaction cleared
Copy of your pre-approval reference number, brought with youSome facilities will ask for this directly
Discharge summary or visit notes, if applicable

Submitting your reimbursement claim

Completed reimbursement claim form from your home provinceAvailable from your provincial health ministry website — form varies by province
All original receipts attached (keep copies for yourself)
Pre-approval reference number included on the claim form
Claim submitted within your province's filing deadlineCommonly 6-12 months from date of service — confirm your province's exact window
Copy of the entire submitted claim kept for your own records
Tip: Set a calendar reminder for your province's filing deadline the same day you receive care — this is the single most common reason valid claims get rejected.

If your claim is delayed or denied

Follow-up date noted if no response within the stated processing window
Written denial reason requested, if declined
