Referral Request Script
Use to formally request an urgent specialist referral from your primary care provider — clear, specific, and structured to get a real answer.
How to use this script
Use this in person or by phone with your family doctor, nurse practitioner, or walk-in clinic physician — whoever can issue the referral.
Be specific about symptoms and urgency — vague requests are the most common reason referrals get deprioritized or delayed.

If the request is declined or downgraded
If your doctor doesn't feel a referral is warranted, or marks it as non-urgent and you disagree, it's reasonable to ask directly:
- "What would need to change for this to be considered urgent?"
- "Is there a specific benchmark or guideline being used to make this call?"
- "Would a second opinion from another physician be reasonable here?"
You're also entitled to seek a second opinion from another primary care provider or walk-in clinic if you feel the concern isn't being taken seriously — this is a normal and accepted part of navigating the system, not something that requires special permission.
